what is Assertive Community treatment (ACT)?

The goal of ACT is to stabilize or improve one’s quality of living, increase individual/independent living skills and wellness. ACT is an effective way to deliver structured, intensive services to meet multiple, individualized needs of consumers as they increase their involvement in the community in roles that they define as important and meaningful.

This is a voluntary program that clients must be willing to participate in.

what services are provided in the act program?

o Psychiatric 

o Some primary care  

o TCM (Targeted Case Management) 

o Counseling 

o Peer Services 

o Mobile outreach and crisis management 

Examples of TCM services include (but are not limited to): Access to phone/calendar, Food bank, Shelter, Permanent housing, Employment, Health Insurance, Health screenings/Primary care access, Applications for SNAP, TANF, Legal issues, Women’s Health, etc
